Mentor Help Request Form

This form is for use by SCTX Computer Club members to request assistance from a Club Mentor.  Your completed request form will be sent by email to the panel of volunteer Mentors who will contact you if they believe they can resolve your problem.  Your name, phone number or e-mail address will not be posted on the Web site.

The Mentors are club members who volunteer to help members resolve problems and do so to the best of their abilities. They are not professionals and they provide this service with the understanding that they or the Computer Club will not be held liable for any problems they might accidentally cause or for not being able to resolve your problem.

TIP: Use "Tab" or the mouse to move from field to field on the form. Do not hit "Enter" until you finish filling out the form or it will be prematurely submitted.

• If a hardware problem, is the computer under warranty? Yes      No
  Describe the hardware problem you are having, including any error messages.
• If a software problem, state the name and version of the software and the full nature of the problem.
• If an internet problem, what is your access to the internet? High Speed   OR  Dial-Up
  Describe your internet problem fully and state your Internet Service Provider (AOL, Verizon, Suddenlink, etc.)

    

Clicking "Submit" will send a copy of this report by e-mail to several knowledgeable Club Member volunteers who will attempt to get you a reply within 48 hours.
